Methodist Sports Medicine (MSM) is an orthopedics practice in Central Indiana. MSM staff and physicians requested an analysis of their patient experience and were interested in improving this experience. An initiative for ensuring that new patients feel cared for when inputting their healthcare information was a digital patient questionnaire. The questionnaire could be accessed at the patient's convenience, whether at their home or in the MSM waiting room. Personable language was used within the questionnaire to provide a sense of comfort for the patient.
